JOB PURPOSE/SUMMARY
Summary of role of team :
- By virtue of the persons served, the Health Link Program and Intensive Long-Term Support Programs represent a place where the Center's vision is lived out on a day-to-day basis.
- The severe and persistently mental ill (SPMI) population truly embodies the poorest of the poor and sickest of the sick.
- Therefore, it is the philosophy of the nursing position to live up to the Center's vision by reaching out to the SPMI, regardless of what situation they are in or where they are at in their recovery and open a door of stability through mental health treatment.
- Services provided are team-based and teams are designed to effective coordinate health care services for clients enrolled in Health Link or ILS programming.
- Nursing services are directed toward recovery and self-management of mental illness, provide the consumer / family an opportunity to improve their overall quality of life, provide training in community living skills, and increase socialization through the provision of daily leisure and psycho-educational activities.
- Cultural and ethnic factors are taken into consideration in the delivery of these services.
- Services are not time-limited but designed according to the needs of the individual being served.
- The ILS program is staffed with a minimum of one staff member for every eight service recipients 24 hours per day / 7 days per week / 365 days per year.
- The ILS program provides service to adults who are age 18 and over who have been discharged from one of the state RMHI's or one of the state contracted providers for inpatient psychiatric services.
- The THL caseload may consist of children, adolescents, or adults who meet the criteria for enrollment based on diagnosis, crisis intervention, and/or medical necessity.
Summary of position :
- This position is responsible for the clinical oversight of a minimum caseload of 50 Health Link clients via indirect contacts each month in an effort to provide advocacy, linkage and referral services as needed.
- Staff member will provide coverage of Anderson Co. Nurse Line at 11a and 3p ea. weekday and correspond with prescriber via use of EMR.
- Staff member also provides direct care to clients residing in an Intensive Long-Term Support (ILS) Program.
- Responsible for ensuring the mental health and medical health needs for the 13 clients residing on-site are met via the completion of medical assessments, administration of medications, collection of vital signs, etc.
- Mandatory to remain awake and alert during shift to monitor client safety.
- Upholds center policy and procedures, and CARF standards.
TYPICAL WORKING CONDITIONS/ENVIRONMENT
- This position is an office-based position.
- Staff members are provided with all equipment necessary to perform their job responsibilities in their assigned office at that Intensive Long-Term Support Program.
- Services are provided 24/7 at this facility.
- The role of this position is to provided nursing coverage on a 12-hour shift despite any weather conditions or holidays that arise.
- This position requires frequent community involvement or engagement in group community outings.
JOB DUTIES/RESPONSIBILITIES

COMPENSATION:
- Starting salary for this position is approximately $22.99 - 30.87/yr based on relevant experience and education.
Schedule:
- Flexible to meet the needs of 24/7 program.
- Twelve-hour shifts that rotate weekends for coverage.
Travel :
- This position requires utilizing a personal dependable vehicle to conduct Center business.
- Maintaining a dependable vehicle and certified driver status is a condition of employment.
- F Endorsement is required for this position and must be able to operate company owned vehicle as well.
Equipment/Technical Competency :
- Basic computer skills required, specifically with Microsoft Office.
- Experience with electronic medical record (EMR) preferred.
- Must be able to use a fax machine and phone system.
Equipment/Technology:
- This position does require basic computer skills for timekeeping and use of electronic medical records entry.
QUALIFICATIONS - ILS (Intensive Long-Term Support) & Anderson Co. RN
Experience / Knowledge:
- Current TN State license as an LPN or RN.
- One year experience in psychiatric nursing and/or acute care preferred.
- Must have the ability to present professionally, work within a team format, and have the mental ability to exercise sound, clinical judgment under pressure.
- Must have the ability to communicate effectively (both oral and written) and possess good time management and organizational skills.
- Basic computer skills required; experience with electronic medical record (EMR) preferred.
- Experience with interdisciplinary collaboration and care coordination preferred.
Education / License :
- Nursing degree with course work or experience in the areas of cultural diversity, human development, primary care, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual developmental delays.
- Candidates with valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license valid in Tennessee will be considered.
Physical/Emotional/Social - Skills/Abilities:
- Minimal exposure to biological hazards.
- Frequent exposure to unpleasant odors.
- Hearing of normal and soft tones.
- Close eye work.
- Lifting up to 50lbs.
- Pushing/pulling up to 250lbs.
- Frequent sitting, standing, walking, bending, stooping, and reaching.
- Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, running, and walking.
- Physical de-escalation techniques will only be implemented at Helen Ross McNabb Center facilities, and not in community settings.
- Ability to effectively and ethically coordinate care for children, adults, and/or families.
- Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions.
- Ability to work within a team format to meet positive goals for adults and/or children and interface with other agencies involved in the ecology of the individual.
- Must have mental ability to exercise sound judgment under pressure.
- Ability to exercise sound judgment and effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations.
- Must also have the ability to communicate effectively and possess good time management and organizational skills.
