Role Overview

We are seeking an experienced Nurse Practitioner (NP) with hands-on experience with PCM/CCM programs. This is an on-site role based in the San Francisco Bay Area, with flexibility for hybrid or remote work.

You’ll work directly with patients and neurologists to coordinate treatment plans, conduct administrative activities, and ensure proactive communication that improves outcomes for patients with chronic neurological conditions.

Key Responsibilities

• Provide telehealth follow-up care for patients with chronic neurological conditions, including Alzheimer’s disease, Parkinson’s disease, dementia, and mild cognitive impairment (MCI).
• Supervise and support Medical Assistants in coordinating care activities, promoting effective teamwork, and maintaining clinical excellence.
• Create, document, and update care plans in compliance with Medicare PCM/CCM requirements.
• Conduct regular check-ins and ensure timely coordination with neurologists and other providers.
• Engage families and caregivers to support treatment adherence and lifestyle adjustments.
• Record and submit accurate time-based activity logs for compliant CPT billing.
• Contribute to workflow and process improvement with the Foresight Health care operations and AI teams.

Qualifications

• Active, unrestricted NP license in California (multi-state a plus).
• 2+ years of experience in neurology, PCM/CCM, or chronic disease management.
• Familiarity with Medicare billing codes and documentation standards.
• Excellent communication and empathy when engaging with older adults and caregivers.
• Comfortable using digital health platforms and EMRs.
• Reliable and proactive in care coordination and follow-up.

Preferred Experience

• Direct experience managing neurodegenerative disease populations.
• Prior telehealth or hybrid-care experience.
• Multi-state licensure (CA + TX/AZ/FL/NY a plus).
• Leadership or training experience in PCM/CCM program operations.

Location

Bay Area (San Francisco, Berkeley, Palo Alto).

On-site preferred, with flexibility for hybrid or remote collaboration depending on patient and clinic needs.
