Job Title: Nurse Practitioner – Concierge Medicine

Location: Alexandria, Virginia—in the heart of Old Town

Position Type: Full-Time--Highly Flexible Hours. (Will consider Part-Time opportunities)

We are seeking a clinically savvy, highly motivated Nurse Practitioner (FNP preferred but not required) to join our exceptional team. Key Responsibilities

  • Provide high-quality, patient-focused medical care for adults utilizing a combination of IN-OFFICE visits, HOUSE CALLS visits or Televisits/video visits.
  • Develop and manage individualized care plans for a diverse patient population, often with complex medical needs.
  • Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions.
  • Collaborate with and coordinate care across a multi-disciplinary team, including nurses, wound care specialists, physical and occupational therapists, and physicians.
  • Serve as a clinical leader and communicator to ensure continuity and excellence in care delivery.
  • Maintain accurate and timely documentation in the electronic medical record (EMR) system.
  • Educate patients and families on treatment plans, medications, preventative care, and health maintenance.
  • Coverage is limited solely to patients under your direct care. PTO coverage provided by the group.

Qualifications

  • Current Nurse Practitioner license in Virginia, (with potential licensure to be added for Maryland & District of Columbia)
  • Board certification (AGNP, FNP, or equivalent)
  • Ideally 3 years of clinical experience preferred but not required (preferably in internal medicine, family medicine, or geriatrics)
  • Experience in HOME-BASED care and/or CONCIERGE MEDICINE is an extremely strong plus
  • Valid driver’s license and reliable transportation for home visits
  • CPR/BLS certification
